July AMM Pulse: Thank You for a Fantastic Fiscal Year!

We just wrapped up our fiscal year in June and we want to thank you for your support over the past year. As this year ends we collate data and I want to take a moment to detail two of the most important health measures for our patients: A1c and blood pressure. These two measures provide the 2 most critical tests we do for patients. The outcomes illustrate the life-saving care we provide:
  1. A1c is a measure for patients with diabetes. A score above 7% is indicative of diabetes.
    • 63% of our patients have diabetes or pre diabetes, which is an incredibly high number that our team focuses on. With AMM's care, our patients achieve remarkable results.
    • 88% of patients who started with an A1c above 8.9% improved their A1c score. That is an incredible and life-saving impact. 
    • Overall, 68% of AMM patients improved their A1c score. These are not just numbers and statistics – they are life saving and result in fewer deaths, fewer hospitalizations and ER visits.
  2. Blood pressure is a measure we all get when we go to the doctor's office, but half of AMM patients have hypertension – high blood pressure which leads to a number of life threatening health challenges.
    • 55% of AMM patients with hypertension lowered their blood pressure.
    • 53% of patients overall lowered their blood pressure. A very positive and hopeful measure.

These impact results happen because AMM has an incredible team of medical providers, behavioral health counsellors, a social worker, a lab, a team of committed medical assistants, a dedicated staff, and volunteers. Their collective mission is to improve the health and the lives of our patients. 

I spent the bulk of my career in the for-profit sector and my experience at AMM is just in a different realm. I feel so fortunate to witness the dedication of everyone at AMM.

We are excited to host a LIVE discussion of “Fall into Wellness: Staying on Top of Essential Health Screenings and Vaccines this Season. The event is sponsored by HealthLit4Wake and will be held in the AMM Sanctuary. Dr. Edwin Burkett, AMM Medical Director, will be one of the panelists in the Ask the Doc format. The panel of experts will discuss essential health screenings, COVID-19 and flu vaccines, and back-to-school health.

This month our Community Health Workers attended the South Central Church of Christ Community Wellness Day. Those who attended the fair had the opportunity to receive information from other vendors about health and resources in the community, and learn self defense and low-impact weight training from a certified trainer.
A huge thank you to all of our garden volunteer groups, who helped us grow our garden this fiscal year. With your help we were able to harvest 2,400 pounds of fresh produce that goes directly to our patients.
We welcome both groups and individuals in our garden! To provide ample opportunity for everyone to get involved, AMM's Garden will be open on the last Saturday of each month until September for individuals! If you would like to bring your group please, schedule that here! We look forward to seeing our garden grow this summer! ☀️
